Yes, a signed claim statement/affidavit is part of the claims process.
Sub-$100 claims are handled a little differently. We offer an online verification option, where the buyer visits a website and indicates they did not receive the package.
If the buyer is uncooperative and files a charge back for a sub - $100 claim, we can use a copy of the chargeback, as well.
Please request completed affidavits on all claims as we do not want automatic charge backs on all claims, but we understand the occasional uncooperative buyer.
